Upgrading INN from Ver 2.1 to Ver 2.2.2

Since upgrading to either 2.2 or 2.3 (which should be out before too much
longer) requires a history rebuild, and an overview rebuild for 2.3 or for
2.2 with the storage API, there isn't any simple way to do it.  There are,
however, some basic instructions in the INN 2.3 NEWS file.  Additional
suggestions are always welcome.

Minor updates (such as 2.2.1 to 2.2.2) can be done with "make update" and
then some quick checking of configuration files.
