How do hosting companies do realtime changes?

nsupdate could certainly do it, and would remove the need to re-load 
your server when there is a change. It will not however allow you to 
add/delete zones from your config. Also, if you are into manually 
editing zone files, you will not be able to do this on a zone that 
allows dynamic updates unless you stop the server for the duration of 
the edits.
