[bind10-dev] include severity name and debug level in .mes definitions and log output

Two reasons to consider adding the debug level (like "30") and severity 
name (like "INFO" or "DEBUG") to the .mes definition files are:

1) an admin could customize them without patching real code;

2) the admin could look in it (or nice rendered PDF or HTML) to see what 
debug level and/or severity name to use to even attempt to get the log 
output (to research a problem versus using debug 100).

As for log output, the severity name is already included, but we should 
also include the debuglevel (if DEBUG) so admin can know how to 
reproduce it.
