 At last week's face to face meeting we agreed to work on a basic recursive
 resolver, with no authoritative support, validation, acl/tsig, or cache.
 This ticket is for the early work on this.  It will include a "Dispatch"
 data structure for rapidly associating incoming responses to outstanding
 fetch contexts, and a first pass at the resolution algorithm with some
 things stubbed out for future work.
