Comment:

 Okay basic usage of the cache is in.

 Not sure if we should call this finished yet, we should probably not
 initizalize the cache in RecursiveQuery, but in the Resolver object
 itself, and pass a reference, and I know of a few other (small) refactors
 we could do (like there are two places in the code where error responses
 are created right now, in lib/resolve and bin/resolver).

 But I think the fastest way forward is to get this into master, hook up
 the nsas as well, and refactor the whole thing then (instead of holding
 off nsas until we think this is Completely Done, and then refactor anyway
 because of issues we find hooking up the NSAS).
