 I've modified the design document with the previously discussed changes.

 I've kept rate/num/period separate, rather than trying to calculate
 between them, because -r really represents an upper limit; resource
 limitations may prevent it from being reached, in which case we want to be
 able to exit if either num or period are reached.

 I've merged verbose/debug mask style as suggested.

 I had made the non-option argument mean different things for v4 and v6
 because the usual
 parameter to specify is different for these two.  However, for the sake of
 consistency
 I've changed the server argument and -l as suggested.
