 Additional notes:
 - assuming no one started reviewing it, I've made a few more changes
   (4933a1c)
 - The change from asio::buffer() to asio::mutable/const_buffers_1()
   may need explanation.  See commit log for 4ee02ef and 4933a1c.
   The redundant layer of wrapper calls may be optimized out, but at
   least these changes shouldn't do any harm.
