Modified: libinn.h

Replace the old xmalloc and company prototypes with new macros that handle
passing __LINE__ and __FILE__ to x_malloc, etc. so that just using xmalloc
in source files works.  Add a typedef for the xmalloc error handler and a
declaration of the external variable holding it, and update the prototypes
for concat, xwrite, and xwritev.
