Don't add an extra Xref: to the overview information when cobbling together
an Xref header for an article that doesn't already have one, based on a bug
report by WATANABE Katsuhiro.  Also fix a bug that caused makehistory to
skip processing any articles without Xref headers for which SMprobe to 
determine the newsgroup name and article number succeeded.
