  Update documentation and various files with the new GitHub repository

Git, git, GitHub and its bug tracker are now referenced instead of
Subversion, svn, inn.eyrie.org and its Trac.  A new era begins!

Also updates the URL of the USEFOR mailing-list archive on ietf.org,
and mention that Forrest Cavalier's web site deals with INN versions
up to 2.3.x only.
