  Reorganize and simplify CHECKLIST documentation

- Completely rewrite and reorganize CHECKLIST.

- Add "IN A NUTSHELL" Section at the beginning of incoming.conf, newsfeeds,
innfeed.conf and readers.conf man pages.

- Switch examples of "ckpasswd -s" to "ckpasswd" alone (using PAM).

- Switch examples of "ckpasswd -d" (dbm) to "ckpasswd -f" (.htaccess-like),
much easier to use.
