  Validate buffindexed persistent block references

Check disk block counts and usage, physical block locations,
index-chain cycles and entry counts, and overview data spans before
mapping or returning them.  Grow the in-memory index incrementally so
corrupt group counts cannot request an arbitrary allocation.
