  Harden stored article retrieval and CAF cleanup

Require complete reads, close descriptors on retrieval failures,
validate article sizes and CAF spans, and release CAF resources on
TOC and header-write errors.  Correct timecaf body offsets and give
RETR_STAT iteration results storage for their traversal state.
