pullnews fixes and improvements

[ requires previously posted "optimized mode" patch ]

+ FIX hostnames on command line (implemented, but didn't work before)
+ FIX cosmetic: correctly goes to newline (in non-quiet mode) when using -O
+ FIX saves new counters on -O message refusals (".")
+ FEATURE uses admin-specified ordering of servers in configfile
  useful when using timeouts and some servers are faster/prefered
+ FEATURE lock file is per-groupfile, not per-unix-user
  allows running several pullnews with different config files in parallel
