Determining cause of rejects

If you temporarily switch to nntpsend, it logs the reasons for rejections.
I think Barry has patches for innfeed that have it log reasons for
rejections if you turn off streaming, but I'm not sure where they are.
I'll try to get them applied when I find them again.
